MIGRAINE RISK ASSESSMENT QUIZ
The Migraine Disability Assessment (MIDAS) questionnaire is a brief, validated tool designed to quantify disability caused by headaches. It measures the impact of headaches on your functioning at work, school, and in social activities by assessing lost days in the past three months. 1
The questionnaire consists of seven items. The grading system categorises the total score into Grade I to IV, from minimal or infrequent disability with a score of 0–5 (Grade I) to severe disability with a score of 21 or more (Grade IV).1

This tool is not specific to a type of headache, but asks about headaches in general, making it applicable for various types of headaches including acute or chronic migraine, tension-type headache, episodic headache, and medication-overuse headache.1
By using the MIDAS questionnaire, you can provide your healthcare provider with a clear picture of how your migraines are affecting your life, which can guide treatment decisions. Please note that this guide is not intended to be used as a diagnostic tool and should not replace discussions with your healthcare provider. Any individual information you enter about yourself will only be viewed in the aggregate with other collected responses.
